Add a contact form

"Add a contact form to the Contact page with name, email, phone, preferred date and a message box. Button text: Request a callback."

Every form the assistant adds is a CRM form: submissions become contacts and conversations in your CRM. The assistant will not add any other kind of form.

Variants:

• "Add a newsletter sign-up with just an email field to the footer."
• "Add a quote request form with a dropdown for service type: Plumbing, Heating, Bathrooms."
• "Add a job application form with a file upload for the CV."
